This arrangement was given to me as a photocopy when I was 16. I never played it up to now having discovered it when sorting some music several months ago.
I have no idea what book its from but the registrations are for the Wurlitzer electronic organ model 4500 and 4140 so it may be a book printed for Wurlitzer. It has no copyright information on it either. I'd love to hear from anyone who know what book it's in. I have included the first page as a JPG download.

I found an audio recording made by Lee playing by what sounds like a Wurlitzer electronic organ to probably to go with the book to demonstrate these arrangement's so we know what the intentions of the arranger is which is rare.
